The report must contain factual data indicating possible commission of corruption or corruption-related violations, other violations of the Law of Ukraine 'On Prevention of Corruption', which can be verified.

3. Circumstances of the violation:

3.1 Full name and position of the person who committed the violation (up to 700 words)

3.2 Content of the violation (up to 700 words)

3.3 Date of the violation

3.4 Location of the violation

3.5 Other significant circumstances of the violation (up to 700 words)

4. Attach files to the report (Files must be smaller than 10 MB.)
